Deputy Ken O'Flynn asked the Minister for Health in view of the fact that alleged endangerment of health and safety was both the single largest category of allegation and the largest share of upheld findings in the HSE Protected Disclosures Annual Report 2025, whether the increase in such allegations was escalated to the HSE senior leadership team as recommended by the Audit and Risk Committee; the measures put in place as a result; and if she will make a statement on the matter.
