The National Shared Care Record is a major national Digital for Care initiative that enables authorised healthcare professionals to securely access key patient information across organisational boundaries, helping to ensure that clinicians have access to the right information at the right time to support patient care. The programme supports the progressive delivery of a single shared view of patient information across care settings and is helping to reduce reliance on fragmented information sources.

Data collated for the shared care record can be made available to patients via the HSE App.

The NSCR is now a live national service and is operational across all six HSE Health Regions. Its adoption by healthcare professionals will ramp up gradually as more data and capabilities are added.

Healthcare professionals are actively using the platform to support the delivery of integrated care, especially in settings where heretofore, there was very little or no access to patient data from other healthcare settings.

The service provides access to key patient information drawn from a range of existing health information systems, including appointments, waiting list information, medications, laboratory results, discharge summaries, referrals, vaccinations and other information relevant to direct patient care.

The programme continues to expand on a phased basis. Significant new functionality has been introduced during 2026, including additional appointment, waiting list and referral information. Further releases are planned later in 2026 and during 2027, which will continue to broaden the range of information available through the platform and improve information sharing across the health service.

The NSCR represents a significant milestone in the implementation of Digital for Care and is a key building block towards a fully integrated Electronic Health Record. By bringing together information from multiple systems and care settings into a single secure view, the NSCR supports more coordinated care, reduces fragmentation of information and improves clinical decision-making.

The NSCR complements the HSE Health App, which provides patients with access to their own health information, and the National Electronic Health Record Programme, which will ultimately provide a comprehensive longitudinal health record across publicly funded health services.

Full deployment will be undertaken on a phased basis through the addition of further datasets, organisations and services and will remain aligned with broader digital health transformation initiatives, including the National Electronic Health Record Programme.
