The Minister referred the Deputy’s questions about voiding a Connolly Hospital incident report to the HSE for a direct response.

Deputy Pádraig Rice asked the Minister for Health further to Parliamentary Question No. 300 of 24th June 2026, in which the Health Service Executive confirmed the occurrence of incidents at Connolly Hospital (details supplied), to confirm who authorised the voiding of the National Incident Monitoring System report filed in relation to an incident; the grounds on which that report was voided; if the individual(s) who authorised the voiding had the authority to do so under HSE incident reporting policy; and if she will make a statement on the matter.
