The Minister referred questions about the voiding of a serious Connolly Hospital NIMS report and related patient-safety assessments to the HSE.

Deputy Pádraig Rice asked the Minister for Health further to Parliamentary Question No. 300 of 24 June 2026, in which the Health Service Executive confirmed the occurrence of incidents at Connolly Hospital (details supplied), if any patient safety risk assessment was carried out prior to the voiding of the National Incident Monitoring System (NIMS) report filed in relation to an incident; if no such risk assessment was carried out, the reason a report describing a serious and active breach was voided without one; if patients or staff on the ward in question were informed of the breach at the time; and if she will make a statement on the matter.
